I bought two of these to go on either side of the MOLLE II 3-Day Assault pack. They fit fairly well with some innovation and really expand the functionality of the pack. They don't line up perfectly with the PALS columns on the side of the pack and I just stowed one of the MOLLE straps on the back of the pouch by weaving it under all the rows. In fact, this makes a narrow vertical channel once you've got it on the pack, which I could see coming in handy to retain something like an axe, tripod, or something you could lash onto the pack to prevent it slipping all the way through. I did have to start under the top row of MOLLE on the pouch, so it is a bit unsecured at the top, and if you pull the MOLLE too tight it bunches up there. On the upside, this makes the main pack zipper more accessible. It also sticks out about an inch on the front, but once the pack is full this will probably be unnoticeable. I like how the pouch is wide enough to be useful but still narrow enough and with some internal pockets to make it a kind of super-sized admin pouch. The only thing I would change is making the MOLLE attachments snap instead of the kind where they are just open ended with a folded over part. I think snaps would be more secure, particularly when using in a bit of an unorthodox manner or if you want to attack something else to the PALS on the front. You could just zip tie it in after weaving the MOLLE. This pouch looks like it's perfectly sized to fit the PALS on the front of the MOLLE II medium ruck. When I was shopping for this, I found the photos a little confusing. This pouch has PALS on the front, MOLLE straps on the back, 2-zip closure on top, inside is one deep hook/loop flap pouch with the ATS logo, and one deep mesh sleeve. The photo of the hook/loop looked like this was on the outside of the pouch, but that is not the case, that's where the flap inside fastens. The rest of the inside is just a bag with a drain hole in the bottom. I can relate.
